For most people, riding a unicycle is hard enough, let alone pedaling with your hands because you're upside down. (Should that be called "manualing"?) This fellow is also on a loose rope, not a tight rope, but a very slack one and topping it off by swinging toward the audience and away as he pedals to the right and left. Wow.
